Chance to win a $5000 cash prize
Official Rules can be obtained in Affinity Publishing Group's Millennium Journal or by mail from Affinity Publishing Rules Request, 740 River Road, Suite 203, Fair Haven, NJ 07704 through January 30, 2000. Entries may be submitted to Affinity Publishing Sweepstakes, 740 River Road, Suite 203, Fair Haven, NJ 07704 Attn. : Millennium Promotion. All entries must be received by 6:00 p.m., January 31, 2000.
1) HERE'S HOW TO ENTER:
NO PURCHASE NECESSARY. Sweepstakes void where prohibited or restricted
by law. Sweepstake ends January 31, 2000. You may enter chance
to win a $5000 cash prize by completing by hand-printing your
name, complete address including zip code and day and evening
telephone numbers on a 3' x 5' card, and mailing your entry to
Affinity Publishing 740 River Road, Suite 203, Fair Haven, NJ
07704 Attn.: Millennium Promotion. Entry must be postmarked no
later than January 20, 2000 and received no later than 6:00 p.m.
on January 31, 2000 in order to be eligible for the random drawing
from all valid entries received. The drawing will be at noon
on or about February 1, 2000 and supervised by an independent
accounting firm. Affinity Publishing is not responsible for late,
lost, misdirected, incomplete, damaged, stolen, illegible or
postage due entries, which will be void. Limit three entries
per person. No photocopied or mechanically reproduced entries
will be accepted. Each entry must be mailed separately in a first
class postage prepaid envelope. A minimum of 2,500 entries is required for sweepstakes to be valid.
The sponsor reserves the right in its sole discretion to void any and all entries of entrants whom it believes in its sole discretion have attempted to tamper with or impair the administration, security, fairness or proper play of the promotion
2) PRIZE PACKAGE:
The sweepstake ends on January 31, 2000 at 11:59 p.m. EST. One
winner will receive a $5000 cash grand prize. The winner will
be chosen on or around January 31st, 2000. Decisions of the judges
are final and binding. Odds of winning depend upon the number
of eligible entries received during the applicable entry period.
All taxes on the prize, if any, are solely the responsibility
of the winner. Prizes cannot be transferred or assigned, and
no substitutions will be given. By participating in the promotion,
entrants agree to abide by these Official Rules.
3) ELIGIBILITY:
Sweepstakes open only to legal residents of the United States
and Canada, at least 18 years old. Employees and directors of
Affinity Publishing, its licensees, parent (Bertelsmann AG),
subsidiaries, Bertelsmann AG company affiliates, distributors,
advertising, promotion and other agencies, suppliers and manufacturers
of promotion materials relating to this promotion and all of
their respective parents, affiliates, distributors, advertising
and promotion agencies andsuppliers are not eligible and immediate
family members (parents, children, siblings and spouse) of such
employees or directors are not eligible to win. Void where prohibited
by law. All entries must be made manually, and may not be submitted
through the use of applications or programs that utilize automated
or bulk submission routines.
4) WINNERS' NOTIFICATION: RELEASES:
Winners will be notified by mail with 15 days of the drawing.
All winners must execute an Affidavit of Eligibility and Liability
Release, and where lawful, a Publicity Release before receiving
prize confirmation or prize. If the Affidavit and the Releases
are not returned within seven business days of notification,
or prize notification is returned as non-deliverable, or selected
winner cannot by contacted, is ineligible or fails to claim the
prize, the prize will be forfeited and another winner will be
selected.
Entrants, by participating, agree to release and hold harmless Affinity Publishing, its licensees, its parent Bertelsmann AG, subsidiaries, affiliates, advertising, promotional, and other agencies and their respective officers, directors, employers, representatives and agents, from any and all liability, claims and actions of any kind and will be held harmless by winner for any injury, loss or damage to winner or third party due in whole or in part, directly or indirectly by reason of the acceptance, possession, use or misuse of prize or participation in this sweepstake.
Winners agree, except where prohibited by law, that Sponsor may use winner's name, picture, portrait, likeness, and voice and statements made by winner for advertising and promotional purposes without further consideration.
